If you are applying to the University of Michigan Aphasia Program (UMAP), please click here.

To apply for other programs at UCLL, an Application Form (pdf) needs to be completed. If your child has had any other therapy or evaluations completed at school or by another specialist, please include a copy of the report(s) with the application.

Please complete and mail the form(s) to:

We will review all submitted information and appropriate candidates will be invited to participate in the programs.

Available spaces are filled on a first-come, first-served basis in response to the invitation.

HIPAA
