Enroll Your Child in PACT

To apply for PACT, download the Application Form (pdf). If your child has had any other therapy or evaluations completed at school or by another specialist, please include a copy of the report(s) with the application.

Please complete and mail the form(s) to:

We will review all submitted information and appropriate candidates will be invited to participate in the program.

Available spaces are filled on a first-come, first-served basis in response to the invitation.

HIPAA
We are committed to providing quality health care which includes respecting patients' rights to maintain the privacy of their health information and ensuring appropriate security of all protected health information. To that end, every person that has access to clinical information must undergo Health Insurance Portability and Accountability Act (HIPAA)  training.
